The Julian Center offers community outreach services to counseling clients. The case manager and client complete a Family Development Matrix to assess thirteen different components of the client’s situation to identify areas in which the case manager can offer referrals to community resources. The case manager is trained to help the client to complete a safety plan and, if necessary, to file a Protective Order with the court. Often case mangers assist clients in filling out an application with Department of Family Resources for Food Stamps, TANF, and Medicaid. Case management focuses on obtaining medical insurance, employment, housing, legal services, clothing, transportation, and child services. The goal of case management is to help create long term self-sufficiency and independence for the client and her family. Through the case manager the client can receive access to Julian Center services such as the Legal Department, food pantry, Thrifty Threads and various other programs as necessary. |
